  • Last three years of personal tax returns

  • Last two years of payroll 

  • One of the following:

    • 2019 IRS FORM 940

    • IRS FORM 941

    • an option of 2019 w3s or W2s for a minimum of two employees, need two years of ptr 2017 and 2018
